Child’s birth certificate

Proof of general immunization requirements through the DH680 form (DOE Code 1)

Proof of a physical examination by Florida licensed health care professional (Less than one (1) year old, on Florida School Health Form)

Any additional documents (if available): IEP from another school district, or any speech, language, therapy, or medical reports.

Already have children enrolled? You can simply add your new child to your existing FOCUS account.

Upload all required documents listed in STEP 1 within the online application in the registered Focus Parent Portal account.

Early intervention screening questions are included within the enrollment form in the Focus Parent Portal. The ESE Department will follow up with families.
